Undergraduate Advising Form Protocol

These forms must be submitted, in-person, to the Undergraduate Advisor during drop-in advising hours.  

Please note that entry into all MATH and MACM courses, excluding MACM 101, is via the Department of Mathematics.  All inquiries related to MACM 101 must be directed to the School of Computing Science.

Course/Exam Conflict Waiver Form 

You must complete this form if you wish to enroll in a MATH or MACM course that has a schedule conflict with a course in which you are already enrolled.  Conflicts are typically caused by time or location conflicts of lectures/tutorials or exams. 

Prerequisite Waiver Form

You must complete this form if you wish to enroll in a MATH or MACM course for which you do not meet the formal prerequisite.  Please note that prerequisite waivers are typically not granted for 100, 200 and some 300 level MATH or MACM courses. 

Third Time Permission Form

You must complete this form if you wish to take a MATH or MACM course for a third time.  Please note that the deadline for this form is 2 weeks prior to the first day of classes of the term in which you wish to complete the course.  This application is not guaranteed approval.
