Be advised that we are in the process of digitizing the various forms that you may require. Those that have been prepared are available here.


Expense Claims

*IMPORTANT - There have been quite a few Expense Reports (Claims) returned to the Department from Research and/or Finance, due to missing information.  To help eliminate these returns and speed up the processing time, please print the attached PDF Form as a reference.  It contains a quick list on important info needed when submtting a claim, along with a check list of what to attach and also a short questionaire that needs to be filled out and submitted with any RESEARCH Claim Forms.  

  • Expense Claims for Employees ONLY If you are unable to create a new expense claim, please send your name & employee number to Casey @ casey_bell@sfu.ca so that you can be set up on the system.

 

Payroll Info (documents)

If you are NEW to SFU Payroll, you will need to print and complete a Personal Data Form along with the Federal and Provincial Tax Forms and hand them into the appropriate person (If you are a Graduate Student, please hand forms into the Graduate Secretary, Diane Pogue.  All other employees can hand their payroll forms into the Financial Clerk, Casey Bell in the General Office).

Along with these documents, you must also hand in a preprinted void cheque, before you can be set up on Payroll. (if you have a Working Visa, a copy of this must also be handed in)
