Application Forms

You can apply using the Online Application Process. Note that you will need to pay an application fee of $90 CAN (students with Canadian transcripts) or $125 CAN (students with international transcripts) by credit card (MasterCard or Visa).

In order to consider an application we require:

1. a completed university application
2. OFFICIAL ORIGINAL TRANSCRIPTS: hard copies should be mailed, in sealed envelopes, directly to the Dept address below. Any up-loaded transcripts in your application will be considered unofficial
3. THREE LETTERS OF REFERENCE: the referee system is on-line. When you submit your application, your referees are e-mailed a secure line where they are able to enter or up-load a confidential reference for your application
4. TOEFL or IELTS scores for applicants whose first language is other than English. Original documents required! (A photocopy is acceptable in the interim.) SFU Institution Code for TOEFL is: 0999
5. APPLICATION FEE: $90.00 CAN (students with Canadian transcripts) or $125.00 CAN (students with international transcripts) - Credit Card only (MasterCard or Visa)

Recommended but not mandatory: GRE General Test Results and GRE Mathematics Subject Test Results. The Graduate Record Examinations provide standardised tests of skills relevant to graduate students. These allow us to better evaluate your application.
